All patients under 18 years old must be accompanied by their parent or guardian.

Please bring a list of medications you are currently taking.

Insurance Information:

• We accept private patients as well as patients with various dental insurance plans.
• It is occasionally difficult to initially accurately calculate patients' insurance co-payment. Many insurance companies will not release fees until after the completion of the treatment. Processing those claims can be complicated and time consuming.
• To minimize your waiting time we ask to provide your insurance information prior to your visit. It is important to keep all information about you and your insurance current. Please keep us informed of any changes to your insurance.
